A Uniform Report Number (URN) shall be voided if it was drawn in error. A Uniform Report Number Void form (SH-R-474) shall be completed when an URN is voided. The SH-R-474 shall be reviewed for accuracy and completeness and approved by a sergeant or above. Once the SH-R-474 is approved, Los Angeles Regional Crime Information System (LARCIS) shall be updated, and the original SH-R-474 shall be maintained in the station/unit file.
