Disability Management Compliance Unit

A

Prepare an original and one copy of Form SH-R-426, Hazardous Materials Injury/Toxic Substances/Communicable Disease Exposure Report for each Deputy involved and submit both copies to Return to Work Unit.  Use original URN.

If a Reserve Deputy involved, also send a copy to Reserve Forces Bureau.

Additional copies to meet Unit needs.

NOTE:  Custody Division personnel refer to Custody Division Order #53.
